Quarterly Certification Forms
The new Quarterly Veterans Certification form is now available for download in Adobe Acrobat (PDF) format. Click on the link below to access the form. You have the option of (1)printing the form immediately or (2) completing it first in your web browser and then printing it.
Intake Form
This form is used when a student is starting their benefits at UCI. This contains a checklist of the paperworks you will need to turn in according to the Chapter benefits you will be receiving. Unfortunately, this is not currently updated with the Post 9/11 GI Bil. If you are a Veteran follow the Ch30 column and if you are a dependent receiving Ch.33 follow the Ch35 column.
